Filing AOS packet for F2B I-130 under 245i. concurrent I-485,I-765 (EAD) and I-131

hi, my original post got somehow screwed up so im reposting.

i already started few threads but have a couple additional questions.

i was finally able to get all the required documentation for a complete (i hope) AOS packet.

since i will be filing all these form concurrently with I-485 supplement A (245i as a grandfathered beneficiary of father's LC filed before april 2001) do i need to include the supporting documentation (fathers original approved LC) with every form ? since they will be in the same packet i'm not sure if i should include 3 copies of the same document. i'm asking becuase I wonder if the whole packet will be inspected by one officer or if it'll be split up and send somewhere else where I guess the supporting doc would be very helpful.

this bring me to my next question if i should include a small one page letter with every application explaining the circumstances of my case and if this also should be attached to every application. I485, 765 and 131

as far as form 765 (EAD) in Q11 it asks if I ever filed for EAD. my father when he initially filed AOS for me (which was denied due to age out) also filed for EAD which i received and it was valid for one year. I will make a copy of both side and include that and answer yes to that question , however i'm not sure in which office it was applied for. can i leave that blank ?

lastly do i need to include a index page listing all the documents included in the whole packet for easier packet searching ? also is there anything specific i need to write on the envelope when mailing the AOS packet out besides FROM/TO? Anything specific as far as the kind of enveloped to use ? and should i use tracking #, signature confirmation when mailing out ?

since i'm filing based on approved I-130 is this the correct mailing address ?
