So, according to my doctor who i went to get a letter off i had one isolated case of "intrusive thoughts of self harm" but I didn't I did however during when i went to ask for help managing my anxiety last year said during a panic attack due to not being able to breath " i think i'm going to die" obviously being rather melodramatic cause by having asthma but never having an asthma attack so it was a sensation I had never experienced before.

This was apparently displayed as an intrusive thought of self harm which I didn't know about.

I answered No to self harm thoughts as i generally forgot this situation happened and didn't know that is what it was told as

I have attached the bit of my letter to the post and the response in email in which i was asked to do explaining the situation

I'm now scared I've ruined everything because I didn't know or remember the situation it was over a year ago

I attended my medical examination on the 24/10/16.
During this medical I proceeded to say , I have never had thoughts of personal self harm However due to a possible contradiction, My doctor has said during a panic at the doctors when I was initially treated I apparently mentioned, during panic attacks "I felt like I was going to die" this was displayed apparently as an isolated incident of intrusive thought/self harm thoughts, although I stand firm in confirming I have never had thoughts in order to harm myself or others nor do I have any history of any active self harm.
The only thoughts that I will confirm I had where when I had panicked I felt was going to have an asthma attack leading to the sensation of being unable to breath, causing me to be rather melodramatic saying I felt like I was going to die.
I do apologize and hope this clears this up and will not cause any trouble, since this was a situation That had not occurred to me as I never thought to harming myself and was so brief I had honestly forgot this had happened due to it being well over a year ago of occurrence.
I however have been given confirmation of no re-occurrence of any sort as seen in the attachment.
