IR1/CR1 Medical Questionnaire

Hi

I have my medical this week, I have all the required evidence and vaccinations but there was one part of the questionnaire which I just want o get a second opinion on.

The section which ask: Prior Country. I was last staying inI the states on a tourist visa from Autumn 2011 until Summer 2012. I know this sounds like it should be simple but I don't want to get it wrong. Should I put 'US' in this section. I technically wasn't a registered resident, so was a little unsure if I should state either 'US' or 'Not applicable'.

Also, In the section which asks: Have you ever been hospitalzed? I haven't ever spent the night or been rushed to hospital etc but have suffered from Asthma issues/Minor attacks resulting from chest infection, 15 or so years ago but was only in hospital for a few hours. so do I mention this or just put no?

first question: you should write down US even though you're not a registered citizen, because you should indicate where you've lived.

second question: you should answer No since you were never hospitalized, because hospitalized means you've at least spent one night at the hospital.

Just to make sure I've understood, I was travelling the states for a year from 2010 to 2011 and then based in one place during the dates mentioned in my first post. because I was on a tourist visa and not technically a resident, I just wasn't sure.

Just out of curiosity, how did you get a tourist visa for almost a year?

Meant to add - re hospitalisation. That would be if you were admitted onto a ward. Going into A and E, being treated and then leaving doesn't count as hospitalisation. Only if you were actually moved onto a ward.

Hi

I initially had a 6 month tourist visa but applied for 6 month extension whilst I was in the Us. which was in turn granted. You can only apply for once, meaning at the end of the extension, you have to leave.
