Does anyone know whether or not you should type in all capital letters on the forms if you do it online? It seems silly to ask, but I've filled out other types of forms for whatever the case was and sometimes they'd ask specifically to have everything capitalized.

Did anyone have issues regarding fitting in addresses when typing online? Also, when it comes to selecting a state for my fiance, it brings up USA states. I'm sure we can abbreviate some of her information, but for something like this, I'd like to make sure everything we can say can go on there.

If there isn't enough space to type, try printing the form and fitting what's left writing it down. If still there isn't enough space, you attach a supplement page.

Which question is asking for her state? I believe you have to leave it blank for her, but can't be sure without knowing which item is it

Sounds good. And for the example forms I've seen, it looks like state is left blank. It was for the section for her address and etc. I'm confused though by you saying leave it blank for her. This doesn't get mailed to her. This goes to the Lockbox in Dallas. I know I can and will (required?) mail her the same exact package for her records.

I meant if it's asking a state for her address and the only options are states in the USA, you should leave it blank. (She is foreign, therefore she doesn't live in a US state)

Not required for you to send her a copy of the package, but I strongly suggest you scan everything and put it on the cloud where both of you can look at the documents for future reference.
