1. I obtained my permanent resident status through marriage to a US citizen over 5 years ago. For the first question of the form should I check "has been a permanent resident for at least 5 years" or "has been a permanent resident for at least 3 years with marriage to a US citizen"? because they both seem apply to my case. However if I check the second option I will need to provide evidence of my spouse, marriage certificate, tax return and other related documents. I want to apply in the most straightforward way, can I check the first option instead?

2. For some questions which don't apply to me or optional questions, should I leave them blank or answering them? For example, name change question, can I leave it blank or do I have to check "No"? For prior marriages and children question, which neither apply to me, can I leave them blank or do I have to write "N/A"?

3. Should I write in uppercase for all entries (name, address, etc)? or capitalize only the first alphabet of each line?

4. Should I write my A-number on the top of every page? There is a place for that but it's not available to write anything in the PDF file. I guess I may need to hand write it after printing it out?

5. The current N400 has an exp. date of 9/30/15, I guess it's okay to use since the newer one has not come out yet right?

1. Go for the 5 year option, it is easier and less paperwork

2. Generally best not to leave things blank, as they may think you ignored/ overlooked the question; put "none", "n/a" or jsut a little cross as appropriate.

3. Doesn't matter, as long as it is legible. I used all upper case.

4. If there is a space for it, they want it; hand write it in if it doesn't auto-populate when you enter the first one.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

If the residence and employement are current, the "To" date should be written as "present" right?

And this one is similar to what I asked before, if I answer "no" to jail, can I leave question 29 in part 11 blank? or do I put "n/a" Because the question already says "if you answered "yes" to ... complete the following table", so I assume if I answer no then it's safe to skip it.

Also for the list of trips outside of U.S., it says to start with the most recent trip, however my most recent trip involves more countries that can be fitted in the space provided, so can I instead list everything on a saparate sheet of paper and just write "see attached" in the table provided? Because if I try to include those trips that I can fit in the spaces they will not be in a chronological order.

Yes, "present" is fine.

No need to write n/a for sub-questions if the main question is answered.
