I-485 RFE Help

Need some help and clarification on the RFE we received. We sent in the DS-3025 and were not required at the time (according to the panel physician in her country) to get the Varicella vaccination or provide the history (of which she has already had it) so nothing was checked on the DS-3025 in that category. I have attached the RFE notice that they sent us if you could please have a look at it and shed some light on a few of the questions that I have below I would really appreciate it.

1. It says to return this RFE with the evidence, since the only address I see is at the top right of this paper, would that be the address we would be mailing our response too? It also states the same address on the envelope that it is the Kansas City Field Office.

2. Are they only asking for the Vaccination Supplement on the I-693 to be filled out by the Civil Surgeon? That is what I gathered it only needing that portion of it, and not the entire I-693 packet done over....

Yes that is what I gather as well. We received something requesting immunization and a new medical which is weird but our form looks totally different and ours came from a support center not field office? So weird

Yeah apparently our papers went from the National Benefits Center to the Kansas City Field Office maybe because they are backed up I have no idea..... but since that is the only address on the form and it says to return it to them that is my only assumption of where it would go. Very confusing with much of this :/
