new form question - part 4

On part 4 of the new form where the US Citizen (or person who through whom the applicant gained lawful permanent residence from) puts their information, does one use your maiden or married last name if a woman? It just says Family Name (last name), not sure which one to put and of course their example form has conditional resident being a female, so on that part the husband just used his last name which is the same as his wives..
