Clarification on steps after NOA2

So we've just received in the mail our NOA2 letter for our CR-1. The actual notice date was 21st Dec 2015, but the holidays had to come and delay things..

I was reading LingChe's NVC shortcut wiki, but it's kind of messy. So, I'm just asking to clarify what our next steps are.

1. I guess we should book the medical and get the National Police Checks done now?

2. Other than preparing our documents, can we pre-fill any forms, or will we have to wait for the NVC packets?

3. Altogether, how many other payments are needed and can we pre-pay them now?

4. Regarding the I-864, I (the petitioner) have never earned any income, or worked in the USA before. What do I fill for the I-864?
