1. My fiancee doesn't have a middle name, so on the 129F form am I correct to write "none" in that box (same thing regarding apartment #)?

2. For proof of relationship, neither of us kept any boarding passes, luggage tags, etc. - does anyone know of any way to retrieve this information or is that avenue pretty much a lost cause if nothing was kept?

3. Is the "Affadavit of Support" form submitted in the original pack with the I-129F and G-325A or at a later time?

We wrote N/A - my fiance has no middle name. You can do the same thing for apt.
2. For proof of relationship, neither of us kept any boarding passes, luggage tags, etc. - does anyone know of any way to retrieve this information or is that avenue pretty much a lost cause if nothing was kept?

Did you buy the plane tickets with a credit card? You could get that statment (or debit card - bank statement).

Another way to retrieve proof is if you received miles from these trips or if you used a travel agent - maybe they have proof.

The I-134 form is only for the interview - don't submit it now.
