Need clarafication on Birth Certificate for a Nigerian

We just received a letter stating for a request of birth certificate. The officer claimed that the photocopy copy is too blurry to read.

The original was mistakenly sent in the previous application. The officer is also requestion for the person doing the affidavit to include an official ID and date of birth.

So we now have to go back to get a new one.

We are a little bit confused.

1) IS the person doing the affidavit suppose to state his DOB on the affidavit or will the stamped and attached government ID suffice?

2) IS the person suppose to get the old affidavit with the previous date or should he get a new one with a new Date?

3) They also just gave him a copy of the birth certificate that was just obtained from the hospital. it is signed and stamped with a current date by the hospital.

4) Should we disregard number 1 and 2 and just mail 3. Although they already have blurry or maybe original of 1 and 2

5) Should we just mail number 1,2,3 together.

What should we do?

My uncle was able to get an archive with the previous date from the court

Also, would'nt a new date raise suspicion?

One more question....does the the age declaration page need to have the date of birth of olthe person doing the affidavit or will attaching his official goverment id be enought...

You said in your first post that it was requested that the person doing the affidavit include his DOB and ID. If it is not customary to include it on the affidavit

itself then attach it, or if possible have it put on the paper itself. I don't know what is customary, perhaps google for a template or the official doing it

should know.
