Getting things ready for AOS and IV packages

Timeline

I just had a few quick questions as I'm getting the documents ready to send to NVC! First question is : Do I have to include the AOS bar coded sheet and the IV bar coded sheet in the package? I know there is a bar coded sheet that NVC, the Case number barcoded sheet" but I wasn't sure if I should put this sheet on top and then the checklist sheet and then the AOS bar coded sheet again?

I also read on the NVC Wikipedia page that it suggest sending one big package but putting the AOS in a manila envelop and the IV documents in another envelop. So should I do that? Put the case number bar coded sheet on top, then put the AOS documents and AOS bar coded sheet in an envelop and the IV documents and barcoded sheet in another envelop?
