Last name change

1.I am in the process of adjusting my status,wonder if and how I can change my last name,i am still using my last I would like to use my hisband last name.i used to have a not valid for work social security but it is expired and lost!,so I can not go and change my name at the social security office.On the forms whos last name should I write,mine or my husband?should I write them letter to tell them I would like name change?what should I do?

2.In 1-765 the first question where they ask I am applying for ?what should I say ,I am thinking renewal of my permission to accept employement?I don't know someone help.

3.in number 10 they ask alien #I don't hve,I will give 1-94 number it is 9digits and two separate numbers am I going to fill the whole number or fill only the 9 digits?this is what I mean 123456789 10 ten is separated.Thanks everyone for your time to read and respond.

Social security cards don't expire.

You better go to a social security office with identification and retrieve your number. You will be in the system ... I sure was - even after more than 20 years outside the US.
