G325A Questions

I'm putting together paperwork for a K-1 visa. And I have some questions about the G325A.

1) If I include an attachment for the G325A, does it need to be signed and dated also?

2) Can I include all information in one attachment for the G325A or does each question needs it own attachment?

3) What is the proper way to deal with non-US address formats? For instance, the G325A asks for a street name and number for residences. One of her address literally doesn't contain a street name or number (village-province-state format). I've read the USCIS is aware of non-US formats, but I don't want the case worker to think I was lazy and used incomplete addresses.

-Thanks

I just wrote "SEE ATTACHMENT FOR COMPLETE ADDRESS" in the street address field on the G325A, then made an attachment sheet with my fiance's complete name and address, signed and dated of course. We were approved with no problems.
