Hi

I am planning on sending my application form for Naturalization this month. I have completed my N400 based off of the 5year rule. My new legal name is not on my GC. Do I need to include my marriage lisence along with my divorce decree from previous marriage?

Other items needed:

Copy of GC (front and back)

Photos

Cheque

Anything else? Also I am living in UT. Where do I find out what location I need to send the application to?
