Hello everyone,

I (Petitioner), am in process of filling out the DS260 on behalf of my spouse and have a question. On the section "Personal 1", there is a question as follows:

"Have you ever used other names (i.e., maiden, religious, professional, alias, etc.)? " Yes or No

My spouse had a different middle-name and a surname prior to the marriage. However, after marriage, her middle name changed along with the surname and this is how it appears on her new passport.

When I click "yes", two text boxes open - one to add the "other surnames" used - second to add "other given names" used.

This may have an obvious answer; however, my question is, should I be choosing "yes"? Secondly, for the "surname", should I input her past surname and for the "other given names", should I input her first name and past middle name?

Please help. Thank you in advance.

yes you should pick yes.

In other surnames you need to include any other surnames she had before switching to your surname. So her maiden name at birth and any other previous married names or aliases. Same goes for the given names.
