my vaccination record been washed out by the typhoon what should i need to do then? can somebody help me a big thank you then

When you said your vaccination record, did you mean your personal record from childhood or the record from your St. Lukes medical exam? If it was your own record, that's fine. They will evaluate you for which ones you need. If it was the actual vaccination record from your medical exam, then call St. Lukes and ask for a copy.

St. Luke's = SLEC

Being you are IR/CR-1 you will not need the DS-3025 (officially anyway, but it would be good to have a record of what vaccinations you received) .. as you do not adjust status after entering the USA.

Childhood record is of miminal value, so don't worry about it.
