k1 visa holder to file for aos

Hello ,i m a k1 visa holder, and am flying this month to the usa,i had 2 questions for vj members, please give me some advice on how to file aos.

I have a question on the form i 864 affidavit of support,because my fiancee didn t make enough money ,

i have a joint sponsor,and he was qualified by the embassy, he needs to file the form i 864 ,

does even his wife need to file another form i 864, or we need just him to file it?

And the other question is does he need to send again w2 forms , and tax returns and bank statements ?

My last question is on the form i 693 medical and vaccinations.I had my medical examinations here at my country twice,

because i was put under 221g, and the first ones expired, i just had the second examinations a month ago,

and i did send the documents that the doctors needed ,i had even my vaccinations records that i took from the hospital here on my country,

all the tests came out really good and my vaccinations was all done, and the doctors of the embassy gave me the cd on a sealed paper with 2 pages,

and said do not open and give the cd to the port of entry on the usa, that s what they gave me , nothing else.

Now my question is on what do i need there on the usa , to file the form i 693, is it allright if i just get my vaccinations records that i took from the hospital on my country to show it to the civil surgeon or he needs that ds 3025 form.

The doctors of the embassy never gave me the ds 3025, they just gave me the cd on a sealed white paper with 2 pages and said give this to the port of entry on the usa,and do not open it. I was at the embassy doctors twice, and both times that s what they gave me, they just took a copy of my vaccinations records and gave me back the original sheet ,that s all i have the sheet from the hospital from my country , not the ds 3025 form from the embassy.

Please give me some help, really confused,god bless.

Please someone who knows anything gives me some ideas, because the day of my flight is coming and i wanna make sure i have everything allright ,i was never given on hand the ds 3025 from the doctors of the embassy, they gave me just a disc on a sealed paper to give it to the poe on america.please give me a answer
