What exactly does this mean?"If you only have other surnames to enter, enter the same given names as above. If you only have other given names to enter, enter the same surname as above." Im having trouble understanding this no matter how simple this is,my very paranoid self doesnt want to make mistakes in filling out this form.LOl I am already using my married name.Thanks a lot for the help!

So it says other surnames used (inc maiden etc) so i put my maiden name.The next line said other given names used.So i put my first name plus my maiden middle name.Is this correct?Thanks a lot!

September 2 Called local ssa and was told SSC was mailed/Elis says Green CARD production has begun

September 4,2014-received ssc

September 8,2014- green card mailed/closed on elis

September 9,2014- green card received yahooo!!

Oh my thank you very much!Thats what i put though i wasnt sure haha.Now i can sleep peacefully.BTW you guys have any idea which address of petitioner i should put?He lives here in Phils now at the time of filling out the form but would leave for usa next month.I dont want to put the usa address as he wont be there til next month.

If it says current address, put your address here in the Philippines, if it asks for an address abroad, put your address there in the us.

If I remember right, we only used our address here in the Philippines when we filled it out.

i'm F2B.

i'm done with this form but I did it differently. In the form, for SURNAMES (in the example there were 2 surnames). So in the SURNAME field I simply put my MIDDLE name and my SURNAME (example GARCIA FERNANDEZ). then for GIVEN NAME/s I simply put my first name.

as to the address of the PETITIONER ... it should be the petitioner's address in the US, his or her permanent address. doesnt matter if at the time of filling up the form the petitioner is here in the philippines for a vacation.

correct me if im wrong.

Hi :) NOTED. Did you also notice the surname examples given in the actual form? Because that's what confused me.

May I know how the OTHERS did it?
