Jamaica, I-134 for K1,K2

Would some knowledgeable person please respond..............

1. Under section 3 of the I-134,This affidavit is executed on behalf of the following persons: (does fiancee put my name)

2. Name of spouse and children accompanying (put None for spouse ? then names for children as well as gender and age of course).

3. My k2ers will be travelling with me..do they need separate I-134 or we just need one with me listed as primary applicant and them under children?

4. No. 9 Previously submitted K1 on behalf of ( Should the answer be none...or say current I-129F with case no. )

5. No.10 I have submitted a visa petition (Should the answer be none...I see where some ppl say current I-129F with case no. )

6. Correct way to answer no 11, re intention to support persons named in Items 3.
