I-134 for K2, Jamaica

Hello again

In my research it is still not clear whether separate I-134 are required for K2.

So i have few questions which I would appreciate clear, precise assistance with, thanks.

1. Under section 3 This affidavit is executed on behalf of the following persons: (does fiancee put my name)

2. Name of spouse and children accompanying (put No for spouse ? then name for children as well as gender and age of course)

3. Should there be 3 copies since its me and the 2 children or just one completed as per above?

4. No. 9 Previously submitted K1 on behalf of ( Should the answer be none...or say current I-129F with case no. )

5. No.10 I have submitted a visa petition (Should the answer be none...I see where some ppl say current I-129F with case no. )

6. Correct way to answer no 11, re intention to support persons named in Items 3.
