Hi VJ friends im about to send IV application to NVC this week here are the docs im going to send pls lemme know if im missing something thank you...

Cover sheet

IV application form

Orig birth certificate

Photo copy of passport (front and back page)

Passport size pictures (with name,dob and case no. on the back)

IV application form - this is submitted online

Orig birth certificate - make a copy of it to send as well

Photo copy of passport (front and back page) - you just need a photo copy of the biographic page.

Passport size pictures - If you're referring to Philippine passport size picture, thats incorrect, it should be visa pictures, 2x2 measurement

Make sure you write the case number on all these documents, if your uncomfortable writing it on the original birth certificate, write it on a post it and stick it on the bc
