I spent the weekend looking over the screen shots of the DS 260....thank you Saylin for the Montreal filers wiki!!! I have some questions about how I am to complete the form.



Do I record for all secondary institutions I've attended (including secondary school)? I have attended several. Being a teacher we can take what are called AQ (Additional Qualification) courses at universities. I have 7 AQ courses at 4 different universities on top of the universities I attended for my Bachelors of Education and my BA. I also went to college after high school. Will I need to document all these? Or do I just include those which I received an actual degree?



What about the name of my supervisor for employment? Would this be the person who is responsible for me or the big boss, ie the director of education for my school board or the head of HR? I'd rather not be poking around at work trying to get this info. Any ideas here???



As for scanned documents.....that I submit EP with the IV package. Since I've been married and divorced before, I assume I submit scanned copies of both original/certified certificates, right??



I got a K-1 visa back in 2012 but didn't use it. It is stamped "Cancelled without Prejudice". Should I include a scan of this in my IV package as well?



Lastly, back in 1983 I was on a J-1 visa and worked at Walt Disney World in Florida. Do I submit a scan of my J-1 visa? Should submit a scan of my ssn card too? I have not found any info about this.

I would write anywhere that you attended where you got the degree. You can include the last high school (the one you graduated from) but nothing any earlier.

Your supervisor would likely be your direct supervisor. However, I have used HR heads as "supervisors" because they were still with a company and I changed positions within a company several times. This is up to you. My DS-260 did not even ask for previous school or work.

Yes you will need to scan the original final divorce decree but you do not need to include a marriage certificate except for the one to your current spouse.

I would not include that K1 visa or the J1 visa. You do not need to submit a copy of your SSN either.

Can you share why I should not include K1 visa or J1 visa info?

You do not need to send expired visas. You will need to provide that information on the DS-260 but they don't need to see them. You can always bring the passport that contains the visa to the interview to show the CO if you're asked.
