I am about to fill out the ROM form and do have few questions if anyone can help:

What will you place in the following :

1. Foreign Service Post

2. Name of Witness 1

Name of Witness 2

Also, if one would like to renew ones passport using the consular outreach, do you have to set up an appointment in advance with the consulate? How long does it normally take before one gets the passport? ( need to change my last name to my married name in my passport )

You can note the consulate that exercises jurisdiction over your state (e.g. PhConsulate gen. LA for South Cali, Nevada, New Mexico & Texas). They also stamp that at the consulate when they receive it so if you are unsure about which consular office exercises jurisdiction over your state/county best to leave it blank.

The line that requires you to name a "witness/es" is optional. You can put "none" if you do not have witnesses at your marriage ceremony otherwise write the witnesses noted on your marriage certificate. Attach both your photo IDs/passport & birth certificate, pay the $25.00 fee and you're good to go.

No appointments for pass renewal as far as I know. It is a first come first served basis. If you are changing your name on your pass though, you will have to file the ROM first or it can be done concurrently within the same consular office. If not within the same office, the processed ROM should be forwarded to the consular office that is handling the renewal. They always say to check back for for e completed passport within 2 months but it is usually sooner. Good luck!

For number 1 find the consular center that services your area. Once found then that is the "Foreign Service Post" that you enter in that area of your form. See http://www.philippineembassy-usa.org/index.php?page=consulate-finder-dc

For question number 2 from your marriage certificate you will enter your witness's information as shown on the MC.

All outreach events will require you to register prior to the outreach. Follow the instructions from the outreach PDF found on the website of the consular service center that is responsible for your area. Depending on your consular service center you may or may not be required to do the ROM separate from the passport change. Contact your consular service center or search their website for their requirements as each consular service center is different.

It will take anywhere from 4-6 weeks to receive your new passport. Keep in mind that you will have to surrender your old passport so during the time that you are filing you will be without a passport.
