Letter from biological parent of stepchild?

Hi, I believe we need to get a letter from my stepson's biological mother in order to bring him to live with his father and me. Does anyone know where to find template letters? My stepson is 16 years old. Thanks very much.

I may have answered my own questions during a google search. I found the attached sample, which seems very thorough. Maybe too thorough, if that is possible? Is it better to be safe than sorry? Sounds like some people use simple one sentence consent letters, notarized of course.

I think you did answer your question You can never be too thorough, though. better safe than sorry, like you said. Definitely get it notarized, too.
