four names on passport which is middle name?

Hello vj members, here in egypt the names in id and passport are four names not just 3 the first name and father's name and grandfather's name great grandfather's name, in the application i am suppose to put the first name and middle name and last name as i know last name is the family name which is the fourth name but my question about the middle name which one suppose to be the second or the third or which one?, i would appreciate your help and your answer.

You can either use just the second name as a middle or put the second and third names as middles. My husband put his second and third name as a middle. It was hyphenated to make it work as a double middle ie abdelrazek-abdelmoaty.
