Lost GC

My husband recently lost his GC (terrible, I know). We filed the I-90 form online, and the only evidence they ask for is a copy of the original card. Is this true? I'm so used to having to submit 50 pages or more for every application. Any help/advice would be greatly appreciated and helpful. Thanks

I always provide more than the basics. Here is what we provided (for a mistake made by USCIS on the GC so we obviously still had the original):

1. The original Permanent Resident card.

2. Copy of my Permanent Resident card with dates circled.

3. Form I-90 Application to Replace Permanent Resident card (2).

4. Copy of the I-797 Welcome Notice showing Date of January 16, 2013.

5. Copy of the I-797D showing date of 1/18/13.

6. Notice granting Conditional Permanent Residence dated 1/15/13.

7. Passport pages: Face page, K-1 Visa, I-551 temporary stamp.

8. Copy of my Marriage Certificate.

9. Copy of my Social Security card.

10. Copy of InfoPass appointment.

We also made an infopass appt to get the I-51 stamp in her passport which is good for 6 months because that's how long they said it would take to issue a new one.
