Timeline

I found my wife's DS3025 . According to the post which I cant copy and paste here. For some unknown damn reason. The backside of the form is filled out exactly as the poster says it needs to be so no new medical exam is necessary. Poster says to mail a copy in with the AOS forms. they get copies right? Not originals.

Generally, yes, they get COPIES. I only hesitate because you say you had the complete original sealed medical packet in your possession, which wasn't supposed to happen -- it was supposed to be given to the embassy, and then to USCIS at POE. USCIS was supposed to keep it on file in some huge warehouse. I still don't understand why you have it in your possession. Most of us don't get anything except a bad xerox copy of the DS-3025, and we never see our medical packets.

Given that you have the original medical envelope, I really think it's best to send it in with your AOS forms. (Keep copies if you can.) Is it still sealed or did you have to open it to find the DS-3025?

N400 filed September 14, 2023, same day acceptance and Biometric Reuse notice, Interview on 2/13/24: Passed and same day oath. ALL DONE WITH USCIS.

2013 November 19: AOS, AP, EAD sent (NOA 1 on 11/22/13)
2013 December 17: Biometrics, San Marcos, CA, 2013 December 24: Online status changed to Testing/Interview

2014 January 23: Interview notice mailed (for 2/27), 2014 January 24: EAD card production, AP approval (card received 1/30/2014)

2014 February 27: Interview and Approval, GC in production (card received March 6, 2014)

2024 February 13: naturalization interview (five-year rule) passed, same day oath - now a US Citizen and done with USCIS!

It is still sealed. Wife said she was told she isn't allowed to open it or she cant get in. She seems to think its a medical report. But she really doesn't know what's in it. Its from the hospital where she had her examination and shots taken. It does say To USCIS (vaccination form.) I hope turning it in is ok. I'm going to turn it in with the AOS . The DS-3025 is here. And its a copy. And was NOT in the sealed envelope. Which is partly why I couldn't tell what it is. Well, its got two numbers on it too. Front and back are different. Backside is a vaccination worksheet. That and my wife ain't helping one bit. Until a few minutes ago.

Ok, 10 minutes later. Talking to the wife again. She says she had two sealed envelopes. And Detroit Airport kept the big one. I cant get from her if it was yellow. Furthermore I want to thank EVERYONE , for helping out here on the forum. As these things unfold. I have been relatively lucky compared to some people on here. I appreciate ALL your patience. And time writing answers.

Anyway to tell if the envelope was turned in at the embassy or not. And they gave it back. I'm assuming for now. Detroit did not take it. Or its a sealed envelope for a US civil surgeons eyes only. Isn't she susposed to have a sealed envelope for the Civil surgeon. But it says TO: USCIS (vaccination form.)

I'm wondering if I should just turn my DS -3025 in . As it looks like another medical exam isn't needed.

To verify again. I have a medical report original ,with a passport picture. It says things checked, ect.All ok. I have a sealed envelope, as described before . I have a DS -3025. And a vaccination worksheet printed on the back . Exactly as another poster says it should be. To not need another exam. And I have a yellow vaccination book that looks like a mini passport book. I have an Appendix C paper. Vaccinations to be completed as an adult.
