I am a battered spouse granted a permanent residence last January 2011. Under INA Section 319a - self-petitioning spouse of a U.S. citizen."

So I begin to process in preparing necessary documents or form, I need to know what are the documents I needed to attach in my N-400 application. Please help me I am desperately want to know the answer.

The N-400 instruction cover what you need.

Here's what we sent - modify accordingly since you are self petitioning.

• N-400 (original)
• A photocopy of both sides of Permanent Resident Card
• 2 identical color photographs, with her name and Alien Registration Number written lightly in pencil on the back of each photo
• The inside of the front cover and signature page of my current U.S. passport
• Our marriage license
• Proof of termination of my previous marriages
• Birth certificate of our son
• Consular Report of Birth Abroad for our son
• Our son's U.S. passport
• Tax return transcripts filed jointly for 2010, 2011, and 2012
• Joint lease agreements for 2011, 2012, and 2013
• Joint title to our vehicle
• Joint insurance for our vehicle
• Joint utility bills
• Heath insurance cards showing our coverage under the same account
• Drivers licenses showing our address together
• Summary of life insurance benefits showing spouse as primary beneficiary
• Pictures of us together

As for your question about your (ex) husband's address, just put UNKNOWN. It asks about current spouse...does this mean that you are still married? You got your greencard through VAWA so don't worry about not being able to fill out specifics about him if you don't have the relevant information.
