G-325A Bio Questions

1. I started filling out the G-325A form and had a few questions...

It ask "Family Name", First Name, DOB, City/Country of Birth for

Father

Mother

(Maiden Name)

This is in just one large Box-Am I suppose to answer all of these questions top to bottom for both my mother and father?

2. it ask for File Number with and "A" in the box....I have no clue with this is for? Alien Registration Number maybe? I was born in the USA, this doesn't apply to me?

3. This form is submitted in connection with an application for

Naturlatization

Status as Permanent Resident

Other (Specify)

I need to check other and list K1 Visa correct?

Family name is the current last name of whoever you are naming on that box, the Maiden name for your mother is her last name before she was married.

If you use the form you can download from the USCIS.gov website it will download and use Adobe it will show you the appropriate boxes and let you type where each of the names go for your father and mother.

If you are a US citizen don't worry about A number

This form is submitted in connection with an application for (if you are using it to submit with an I-129F petition mark Other and type the "I-129f") It's so weird because I did one 2 weeks ago and that part wasn't on the form.
