Whichever name she is using legally and known by. Some choose to retain their maiden name, some choose to use their married name. My married name was used on all forms and I changed my passport, DL etc into my married name as well. As Yodrak points out though, if she uses her married name, maiden name will also be on the forms under other names used.

On the I-130 If you are married do you put your spouse's married name or her maiden name.

Assuming that Ninetok's wife is using his surname, for at least some things, because if she wasn't she wouldn't have a "married name" to ask about - both.
