Timeline

Hi quick few questions, is the birth certificate needed for the medical? Are there any other documents I need to take other than the police certificate and passport pic? Thanks.

The list they gave me when I called them last week to make my appointment was:-

Passport

1 passport pic - either UK or US size, they can accept either

completed medical questionnaire from the website

vaccination records

information from your gp about any medication you're on

copy of the police certificate

At her appointment in July, we brought

er passport for ID purposes,

a passport size photo,

the form filled out from the website (although they will give you a form to fill out when you get there anyway)

Copy of the Police Certificate

Shot Records (even though her GP said she was good on shots, they said she'd be required a new TB shot so we got it there)

And Nich-Nick is absolutely correct about Money. It's more expensive than either the visa fee or the I-129F fee but thankfully cheaper than Adjusting Status

great thank you. I have my vaccination records but not sure if what I have had in the past Is what they are after. so by any chance do you know which ones they want people to have had?

As per usual Nich-Nick is correct. The only thing you may want to check is that not only have you had the TDAP, but you've had it in the last 10 years. If not you need a booster. At least that's what they told my fiance at her exam, but it was only £35 to get it there and it means we shouldn't have any hassle at the AOS so for us it was worth it

Yes I just realised that today. I do need my TDAP. so i'm gonna call my doctors tomorrow and see if they will do it for free. I'm getting an MMR for free next week so hopefully the TDAP will be too. thanks

Tdap is tetanus, diphtheria, and pertussis. If you have had a pertussis jab at anytime, you can just get the Td as a booster if you haven't in the last 10 years.

And make sure they give you the Pertussis version of tdap not the polio one.

What happens if you have the polio version and not pertussis? Will you need pertussis as well?

I had the polio version last week...got my medical tomorrow so I'll let you know what they say. I really hope so bad I don't have to have it again :o

What happens if you have the polio version and not pertussis? Will you need pertussis as well?

The requirement is pertussis for whooping cough. That isn't the same disease as polio.

If you have childhood records chances are you have pertussis in there. In the UK they give combo shots. The 4-in-1 is

DTaP/IVP which is Diptheria Tetanus acellular Pertussis/Inactivated Polio Vaccine.

There is no shot that mixes only diphtheria tetanus and polio named by only the letters T, D, and P. There is the Td/IVP. But that shows the Td which is on the required list and the IVP part for polio is just extra not needed. I think having that Td part might get you by, even though you really should have had the pertussis accounted for sometime in your life and the Td is more of a booster.

What is the exact name of the shot as written on the paper you are taking in?
