Alright, I am probably over-thinking all this but better be safe than sorry, I guess..

I am the beneficiary. I'm filling out all the forms, since my husband has been just crazy busy...

Ok, so here I go:

I-130 FORM:

1) Question 17. I put my husband's name, Relationship- husband. DOB etc.

But I (the beneficiary) have no children. So do I write down "No children", or leave all remaining lines blank?

2) Question 15. I am a self-employed private english teacher. So-- "Self Employed Private English Teacher", employment began on xx/xx. Correct?

3) Question 20. There is a diacritic mark (ó) in my street address. Does it count as a non-Roman letter?

If not, do I write N/A?

4) Question 22.

On the RIGHT -- WARSAW, POLAND.

What about City and State on the left? N/A or leave blank??

G-325A:

Q: Alien numbers.

NONE or N/A ?

Q: Current husband or wife (if none, so state)

Family name (for wife, give maiden name)

There is only ONE line in the box, so on my form - HUSBAND .................. SMITH

and on his form: WIFE ............................NOVAK

Correct?

Q:

Last occupation if not listed above.

If it was already listed above, do I put: "Already listed above", "None" or "N/A"??

3/31/2014 -Approved 5/21/2014- AOS fee invoiced and paid

5/22/2014- AOS package overnighted to NVC

5/28/2014- AOS scanned into the system

6/20/2014- IV invoice email (but fee still locked on CEAC )

6/23/2014- IV fee finally unlocked and paid.

IV package overnighted to NVC

6/30/2014- IV scanned in. DS260 completed.

7/1/2014- AOS accepted

7/2/2014- False checklist (AOS reviewed)

8/1/2014 False checklist (attorney's G28 reviewed)

8/2/2014 False checklist (NVC not sure why it was generated)

I am the beneficiary. 8/12/2014 CASE COMPLETE!

8/26/2014 Medical

9/2/2014 Interview... APPROVED!!!

9/5/2014 Visa in hand

Removal Of Conditions:

10/29- Package sent to VSC

11/1 - NOA1

11/17- Received biometrics appointment letter

11/30- Biometrics appointment

Just leave the rest of the lines blank

Sure, though I don't a reason for the 'self-employe'd part. Private English Tutor sort of explains that

If not, do I write N/A?

I'm not sure but what would it hurt to write it there properly anyways? It wouldn't.

4) Question 22.

I wrote N/A N/A

Correct?

On the HUSBAND'S form, you will fill in YOUR maiden name.

On the WIFE'S form, you will fill in your husband's last name.

Seeing as I don't know your last names, I can't tell you if that's correct.

Q:

I just left it blank. I suppose it would probably have been better to write 'Listed Above' or something similar

What I meant was whether a word "husband" and accordingly- "wife", should also be written in that same box?
