Proof of citizenship for the 129-f shows to be Copy of the Birth certificate (front and back) for the US Citizen or a copy of ALL pages of the US Citizen's passport issued with a validity of at least 5 years

So does this mean has been vaild for atleast 5 years .........or will be valid for the next 5 years ?

The reason i ask is because the city i was born in is about 1800 miles away,and they require you pick up a duplicate in person. Also it has my mothers maiden name as my last name , which im sure will open up another can of worms. I have only had my Passport for 1 year, but have been out of the country twice, i would think it would be enough. I hope !! :unsure:

If your name is different on your birth certificate from your name on your passport, you will need to attach a letter of explanation and possibly some legal document for the justification of the difference. It is likely that just a letter will be sufficient. The passport sounds like it has validity for five years if it was issued last year. I don't think you have much of a problem.

You might consider contacting the county where you were born for obtaining a certified copy of your birth certificate. Usually they will send one to you when you send them a check for the cost and a stamped self-addressed envelope. I've never heard of such a requirement about appearing personally for a certified copy. Maybe this city has been implementing some stronger rules since September 2001.
