I have a question on Form DS 156K. Again for a reference that both my fiancee and I are divorced so we are required to give information regarding our past life.

This brings me to the Question 4 on the form which is a bit confusing since we are divorced

4. Marital Status

a Name of Spouse

b Date of Marriage

c How and when the marriage was terminated

d If presently married... How will you marry US Citizen....

What should be write?

Do I write something like this

4. Marital Status - Divorce

a. Name of X-Husband OR NONE?

b. Depends on what answer is given to part 4a

c. This can be explained in our words with the date of termination

d. Not currently married.

Only the beneficiary fills out the DS forms for the interview. If they are divorced, then yes, they fill out the info of their ex-spouse and it asks how and when the marriage was terminated. That is where they will list they were divorced and the date of their divorce.

So here name of Spouse will be the Ex-Spouse? Just re-confirming... Yes beneficiary is filling out the information.

c. How and when the marriage was terminated (Date of Termination and Ended with Mutual Consent?)

d. Currently not married (Divorced).
