Do we need G-325A for both of us again, or only for my now husband?

On the G-325A he is filling out, on the line for wife, it says "FAMILY NAME, IF WIFE GIVE MAIDEN NAME". I was married once before but I still go by my first husband's last name. Would I give my actual maiden name (given at birth) or use the name from my first marriage?

You only need the applicant's G-325A with form I-485.

The beneficiary is the only one who submits the G325 for the AOS and we used Claudeth married name.
