Filling out i-30 form, last/family name?

I married my husband on February 14, all of my documents including passport, driver's license, birth certificate have my maiden last name.

I cannot get any of them changed to his last name officially yet but I would like to get it changed eventually.

Should he use my maiden last name on the i-30 form or should he use the last name I will be assuming?

I would get your passport changed before doing this. Regardless of what name you use on the forms your visa and green card will only be issued in the name that is on your passport. If you want to use his last name have the passport updated then go about the rest of it because its very expensive to change the name on a green card after you already are issued it.

Yes but in that instance bring both your passports to the interview just in case they ask.
