I ended up having to pick mine up from our local post office as they said the city was not correct and therefore could not deliver it. It was there about 2 weeks from the date the DOS told me it was sent. I would try there first and if not then you will have to pick it up from the Embassy.

We had to pick it up 15 days before our appointment. They will not allow you to pick it up before then because they want to give the unreliable mail time to arrive.

I'm sorry, my first reply was insufficient. We picked ours up from the embassy 15 days before our appointment because Dominican mail never delivered it to us. Search the Santo Domingo Embassy website for "k-1 visa instructions" to see a sample packet 4. Yours will be a little different depending on your specific requirements. At the bottom, there is a telephone number you can call from the Dominican republic after buying a PIN from banco popular, an also an 800 number you can call and pay $7 with your credit card from the US. They can tell you the status. If you call 18 days before your interview and you still haven't received it, like ours, then they will give you instructions on how to pick it up, which date an time you should be there, and also to do your medical all in the same day. If you are coming from Santiago, I know a bus that leaves at 3 in the morning, picks you up at your door, and drops you off in the afternoon, for 1000 pesos. Personal Message me if you would like this information.
