I-130 Form Questions

Timeline

We are about to file my second green card (having abandoned status previously).

A couple of questions please.

1. In our original submission my wife (she is the USC) applied for me using her previous name (the name which is on her passport). It is her ex-husband's name. My presumption was that we should use the same name this time. The complication is that she will have to renew her passport by June of 2013. Her intent is to use her married name (my surname) now. Would it be better to file the petition in her current married name now and let them make the connection of her passport info as it is now combined with our marriage certificate and the name on the application being current? We have been married just over 9 years.

2. Question D2 on the I-130. I assume the simplest way to deal with this is to refer to an "Attachment" and then explain everything with a letter attached to the application. Am I correct? I am not sure which specific date they are referring to in the question. Is it the date she submitted the original application? I guess I can explain it all out in the letter related to the various timing. Any suggestions?
