Hi all, I need an urgent answer as to what I should do about the situation here goes.... Back in 1997 this woman misrepresented herself to me and claimed that she owns this employment agency and can legally get employment authorization for me. She filed an I-765 on my behalf I have no idea under which catergory however back then i received a letter from uscis requesting a copy of order of supervision...needless to say i was never under any order of supervision in any form and could not submit the informarion asked..subsequently uscis denied the application I am assuming since the website latest info is the request for evidence and I moved and never received a final decision from them....I am now in the precess of filling another 1-765 being an immediate relative...an wondering how i should answer the question on the form...I know that I should answer yes but should I include seperately an explanation because I dont want them to think that I submitted the original form which i certainly did not... What should I do...

Do exactly what you said. Never misrepresent anything to the USCIS as honesty is the best and only policy with them. Good Luck!
