I'm a newbie here.. Anyone ca help me please??? Regarding DS 157.. I paid my K1 visa last August 31, 2012 in the nearest BPI in my place but the teller didn't give me a validated form of DS 157.. Do i still need to go back to the bank where i made the transaction and ask for it?? thank you!

You just have to fill up and print the ds 157 online..the teller will not give you one...
