You can type out a supplemental sheet listing all jobs and type "See attachment" on the form.

Thanks, i can't copy to put here the form, but i put fist the primary job, then in the line that i have to put the numbers $..... , , Do I have to put all the money of my three jobs , , the total? and in another sheet put the other two jobs names, that is correct? thanks.

Listing the primary job is fine, but you might want to include 'See attachment' if you are including an attachment. You can just put the other two on the attachment, or you can list 'See attachment' and list all 3 on the attachment as I previously mentioned.

Yes, you list the total projected annual income.
