questions about I-765 form Question 16

My background: currently F-1 student visa and applied OPT EAD card 2 years ago

Regarding Question 16, should I put C 9 on this line? Also, where should we send the I-765 form？ we r in New York and will send 485 and 130 concurrently. So I should also send this form to 131 South Dearborn - 3rd Floor, Chicago IL?

My fees can be waived ?? I notice here, on Instructions Page 8, If you filed Form I-485, Application to Register Permanent Residence or Adjust Status, as of July 30, 2007, no fee is required to aslo file a request for employment authorization on Form I-765.

yes, You fee is waived, the Code is C9

The form should be send to Chicago lock box with your 485 and 130 in the same packet.
