Bar coded cover sheet

Can someone please tell me where you got that bar code cover sheet which you must have to send that AOS packet to NVC?

Once your payment shows as 'paid' on the payment portal you can click on a link next to it that says 'receipt' or something along those lines. It'll give you the option to print it out or send it to your email, and at the bottom of that print out you will see the bar code.

go online where you made your AOS payment.Go to Receipt.

at the bottom... print document cover sheet.

You don't really need to wait until status shows as "PAID"

Receipt is available to print immediately after you make AOS or IV payment. It also gives you an option to send cover sheet to your email address in pdf file attachment.
