Packet 3 questions: court records, police records, and name differences

Timeline

The consulate in Montreal has just received our petition and mailed Packet 3 last Thurs so my fiance and I have started working on getting all the required stuff pulled together. As we've gone through this I have the following questions which I haven't been able to find any answers on so far:

1. Court records - Is this only for criminal offenses? Only if convicted? My fiance has only been to court for minor traffic violations and they were thrown out each time.

2. Name variations on legal documents - My fiance's passport has his last name, his birth certificate and driver's license have a De in front of his last name. I wasn't really paying attention and missed this when photocopying his passport. I filed the I-129F using the De but the letter I got from the consulate doesn't include it and matches his passport. Since I know they will look at both the birth certificate and and passport will this cause an issue? Right now we're filling out the Packet 3 stuff without the De but listing his name including the De as an alias.

3. Court and police records - Given that my fiance has 2 ever so slightly different names does he need to get these for both names? If just one which one?

We've tried calling the consulate and been bounced around and told to "check the website" and/or call another number, which just repeats "check the website" but adds "or get a lawyer." I'm going to try email next but so far everyone I've reached through this process has just oozed apathy so I'm not very hopeful. In the meantime any additional insight is greatly appreciated!

3/14/12: I-129F NOA2 (Approved!)

3/20/12: NVC Received Petition

3/29/12: Petition Left NVC

4/19/12: Consulate Received Petition

4/30/12: Packet 3 Notification Received by Petitioner in US

5/11/12: Packet 3 Received by Beneficiary in Canada

5/28/12: Packet 3 Returned via DHL

6/5/12: Packet 3 Received by Consulate, said they'd emailed Packet 4 (haven't received)

8/2/12: Consulate Interview at Montreal
