Form DS-230

Q's #30) List below all places you have lived for at least six months since reaching the age of 16, including places in your country of nationality.

Begin with present residence.

Little background info:

1) The beneficiary has lived in her birth State (HP, India) since she has turned 16.

2) After high School in her birth State, beneficiary went to college for four years in (KS, India).

3) After college, the beneficiary worked in (Delhi, India).

4) Presently the beneficiary travels and lives back and forth between two State (#1 and #3).

So my questions would be for Q's #30 in form DS-230:

1) Do we need to include beneficiary's location when she was in college for 4 years?

2) Do we need to enter all the above three locations?

3) Will the beneficiary be required to get PCC Passport Clearance Certificate and PCC Police Clearance Certificate from all three location (3 different state).

4) Since the beneficiary still lives and travels back & forth between in both locations (#1, and #3),

for dates, Do we put "Present" for both locations?

5) Do we put the complete address? it only asks for "City or Town"

6) Also is it Okay to have two entries for the same Location? (Location #1)

(with most recent first.

Location #1 = whatever date to present (back to hometown state)

Location #2 = whatever date to whatever date (work years in diff state )

Location #3 = whatever date to whatever date (College yrs in diff state)

Location #1 = date turned 16 to whatever date (in hometown state)

7) So since the beneficiary has lived in 3 locations more than 6 months since 16 yrs of age,

Do we need to get 1 Passport Clearance Certificate from hometown state

and 3 Police Clearance Cert? 1 from HP, 1 from Delhi, and 1 from Mangalore?

Thank you guys !

Passport Clearance Certificate will only be issued from the state/city from where the passport was issued. For a Passport Clearance, please ask her to google Passport Seva Kendra and go to their website and see if her city has a PSK. If there is one, then she'll get her Passport Clearance in a day.

For a Police Clearance Certificate, if your passport was issued within the last six months (it could be renewal, re-issue, etc.), then I believe you only need a PCC from your current place of residence. If not, yes, the beneficiary has to get a PCC from all three states/cities.

I'd say it's better to declare than to hide.

2) Do we need to enter all the above three locations?

Yes

3) Will the beneficiary be required to get PCC Passport Clearance Certificate and PCC Police Clearance Certificate from all three location (3 different state).

Passport Clearance only from one state but Police Clearance from all three

4) Since the beneficiary still lives and travels back & forth between in both locations (#1, and #3),

Is she working in either of these places? If yes, then you can put the place where she is working as present. If not, then I'm not sure how to go about it.

5) Do we put the complete address? it only asks for "City or Town"

No you dont. Just put the city/town and the state.

6) Also is it Okay to have two entries for the same Location? (Location #1)

Yes

Location #1 = whatever date to present (back to hometown state)

Yes, and yes.
