When I filed AOS for my previous wife who came on a K1 VISA, I filled out the application with the name I wanted to appear on her green card, It was her first name, middle name (Family name, that is how they do it in the Philippines) and then my last name. If it is just the cover letter, you could use the name on your K1 VISA, but the AOS petition should show the name as you want it to appear on your green card. Don't know if this answered your question, but I tried. The A# is also tied to your maiden name until the AOS and if you chose the married naming format, then it will be switched over to that and the green card will reflect it.

ok got it..I used in the cover letter how it appears in my visa but in the forms I used my name the way I want it to appear in the greencard.
