Please refer to page 5 of the Form I-129F instructions for details. The address looks correct, the check payee needs to be changed.

Yes, the address is correct unless you're planning on using Express Mail or a courier service. In that case, use:

As for the check, it needs to be made out to U.S. Department of Homeland Security, NOT "USCIS" or "DHS." (link)
