When I went to my pannel physician medical exam and they asked me if I had any criminal record I do NOT know why I said no. I had a DUI charge back in 2007 right out of college in California. I guess it just slipped my mind because I am in Canada now and that happened in the states. Anyway I went to my interview and the interviewer said "Oh, you had a DUI charge, you have to go back to your pannel physician and get an evaluation done". So I went back to the pannel physician and they gave me 3 forms. 2 of them were to be filled out with my "Family" physician. Now the problem lies here. When I was in San Diego for the past 10 years I stopped going to the doctors when I was about 16 because I didn't have health insurance. I've only been in Canada for about 6 months now and didn't go to the doctors here. So I went into a walk in clinic at AppleTree medical and the physician did fill out 1 out of the 2 forms with me but made a note saying she only met me that one day.

Has anyone else experienced this evaluation? How long is this process? The Pannel Physician receptionist/office manager who ever she is, is never really helpful. I explained the situation to her and she said "well the whole point of those 3 pages is to get it filled out by a physician that you have history with. Just get it done and we'll go from there." I asked her what the next step was and she just said "I can't give you a timeling or any other information, just get it done."

I've waited 6 months after leaving abruptly from San Diego (long story). This is near killing my husband and I at the moment. If anyone has any advice or help I'd really appreciate it. I'm going to be submitting the 3 forms with the requested information, even though there is some missing, it's the best I can do.
