My fiance has income he earns from an employer and he also makes a little bit being self-employed. Does he have to declare both incomes on the I-134 and how will he add them up on the form?

To make matters more confusing, he got a pay raise this year.

Should he use his new salary from his employer and add in his self-employed money earned from last year? Or should he add the money he earned so far this year from self-employment to this new salary?

Follow the suggestions for both. The income he puts on the I-134 will be based on the total income shown on his tax returns.

He only needs to show that his income meets the minimum threshold income.

So basically just lists his old income as it appears on line 22 of his tax transcripts?

If that is the line for "Total Income". Report the total of all his sources of income before any deductions. If he expects to make more this year, then he can report that amount, but he needs to be able to justify that amount if last year he made close to, or less than the threshold income.
