Hi...i would just like to ask if someone had gone to their local health center and asked for a certification of their vaccination records? I have read in one of the post that her certification w/c she got from the local health health center was denied since it wasn't stated in her certificate the dates of her vaccinations, BUT I have also seen one post in YouTube that she had her certificate from the local health center without the vaccination information...like what kind of vaccine w/ date on it...what was indicated was that the local health center was certifying the fact that she was fully immunized..nothing more...hope someone who have done this certification thing could share what they did and their experience during the immunization interview...so i could basically know what i could request from the local health center what to put in my certification of immunity. Thanks in advance

hi....

got vaccination report from local health center....

i requested that the certification be as complete (data) as possible...

like the dates administered, what kind of immunization, lot number, complete name - suggest with middle name, with my complete birthdate, and signed by the head nurse and certified by the head of the local health center.....

when i had my immunization interview at st lukes, it was easy for them to figure out what i need and they administered it..... remember that there are vaccinations or immunizations that require double dose...... i did mine in a timely manner..... but the kind of vaccination st lukes require vary - depends upon age, exposure, etc.....

suggest you get yours as specific as possible just to be prepared....

they administered only the last dose of two immunizations.....

that way, i have completed the requirement......

as i have mentioned, the shots vary.

depends on age, exposure and other factors....

cant find the link right now where you can research.....

but try browsing the website of US embassy and st lukes......
